Services for Aging in Place

Care Coordination is a service of LOA to help persons over age 60 remain as independent as possible for as long as possible.  Care Coordinators evaluate cases to determine which services are needed and link the person with those services. Another component of Care Coordination is Options Counseling which allows the senior to select and arrange the services of their choosing after meeting with a case manager.

Socialization

The Congregate Meals Program, which we call “Diners Clubs,” began in the 1970’s for seniors who are not homebound and who would benefit from getting together with others. Diners meet at a central location to enjoy an interesting activity or program, a hot nutritious lunch, and the company of others. There is no cost, but donations are accepted.

Care Transitions

Care Transitions involves a Care Transitions Health Coach working with seniors to help them have a smooth transition from hospital to home. Health Coaches ensure that the person is aware of these four pillars: Follow-up Appointments, Medications, Red Flag Symptoms, and Personal Health Record. This service is only offered to seniors for whom LOA receives an insurance referral.

Center for Health and Wellness

Location: 4902 Frontage Road NW, Roanoke, VA 24019

LOA’s Center for Health and Wellness opened in 2022, providing a much-needed new space next to our main office in Roanoke. The Center is the hub of LOA’s Nutrition Programs and the home of healthy aging activities and events. The building houses the headquarters for the Northwest Roanoke Meals on Wheels site, the Star City Diners Club, and LOA’s Nutrition Programs offices.

  • The Outdoor Senior Exercise Area, located behind the building, opened in 2024 and offers fitness equipment designed for older adults to improve strength, endurance, and balance. The exercise area is open to the public Monday-Friday, 8:30am-2:30pm.
